ELSA Parent and Teacher Feedback Forms – Item 564

These ELSA Parent and teacher feedback forms are for reporting back about the pupil’s progress after intervention. It is a great way of keeping the teacher and parent informed and giving further advice for reinforcing the learning from ELSA Sessions.

Editable PDFs Overview

Open the PDFs with ‘Adobe Reader’ which is standard on most computers. It can easily be installed if needed and is completely free. These PDFs may work in other readers but these notes have been written based on Adobe Reader.

Text boxes

When you open your PDF with Adobe Reader, you will see blue boxes These are there to show you where you can type. Don’t worry these don’t show if you print your resource. Each text box has a font embedded so you can’t change the actual font. The text size is automatically adjusted depending on how much you write on boxes that need that. So the more you write, the smaller the text will become. Be concise and only write what you need to. KEY POINTS ONLY. This helps keep everything neat and tidy on one sheet of paper.

Save the file

To save the file once filled in, all you need to do is click ‘FILE’ then ‘SAVE AS’ find your folder and name your file.

Print your file

You can print hard copies of your sheets, once typed and keep them in a file  if you wish.

To print your file, click ‘FILE’ then ‘PRINT’, set the size of your paper to A4, check the box ‘fit to the printable area’, then print.
